Types of Guardian Reports

Initial 90-Day Report

Final Report (End of Guardianship)

Initial 90-Day Report

✔ Due within 90 days of guardianship commission.

✔ Includes:

• Activities since appointment

• Discovery of important documents (e.g., will, health care proxy)

• Financial updates (income collected)

• Changes since court hearing

✔ If discrepancies arise (e.g., missing money), be prepared to explain.


📌 Compliance Conference:

• If all requirements 

✔ Due within 90 days of guardianship commission.

✔ Includes:

Annual Report

Final Report (End of Guardianship)

✔ Three main sections:

• General info (Guardian & Ward details)

• Property management (Income, expenses, assets)

• Personal needs (Ward’s health, housing, and well-being)

✔ Financial tracking is critical

• Keep detailed bank statements

• Use financial software (e.g., Quicken) if needed

✔ Attach a medical statement from 

✔ Due annually by May 31st

✔ Three main sections:

• General info (Guardian & Ward details)

• Property management (Income, expenses, assets)
