📌 Why Reporting is Required

• Reports allow the court to track your actions as a guardian.

• You must keep detailed and accurate records.

• Even if you don’t receive a reminder, reports must be submitted on time.

• Late reports may result in a compliance hearing.

✔ Check your local court’s required forms.

✔ Include the index number on all reports.

✔ Notarize your report before submission.

✔ File the original report with the county clerk’s office or guardianship office.

✔ If you have co-guardians, file a joint report. If not possible, file your own.

✔ Keep reports legible (typed or neatly handwritten).

✔ Balance detail – Too little makes things unclear, too much is unnecessary.

✔ Re-read your guardianship order before starting.

✔ Update the court if your address changes.

Who Receives the Report?

Who Receives the Report?

• Court Examiner → Must receive the full, detailed report.

• Other Recipients (listed in your order) → May receive a limited version.


Public Record Warning:

• Do NOT include: Full bank account numbers, Social Security numbers, sensitive medical details.

• Instead, provide partial account numbers (first/last four digits) and submit private information separately as an addendum to the court examiner.
